Avance Care primarily offers care and support services to
semi-independent 16 -18-year-olds with varied needs. This includes young people with challenging behaviour, ADHD and other root causes of disruptive behaviour including ex-offenders and those mixed up in gang affiliation. We deliver care and support for both genders housed in different units.

As part of our method statement, the bespoke care provided begins with a care plan where development and progression targets are set after the needs of the young person is established. we have 2 main areas of focus - education /training and employment pathways.

 

Steps in care provision
Care plan - First 2 week of placement

PEP - First 3 weeks of placement
Updated risk assessment - First week of placement
MISPER Pack - First week of placement

Independent living assessment - Last week of placement
Career pathway - First 3 weeks of placement

Floating support (18 -25-year-olds.

Avance Care provides a step - down service where care leavers of adult age are able to receive continued support and accommodation in a bid to be fully independent and able to access services and entitlements themselves. this is available under the request of the commissioning local authorities who recognised the need for continuous support for an individual. Accommodation provided is usually a house share provision where staff visit and provide 5 hrs or more support key work sessions to service users.

Emergency placements - up to 18 years of age.

In situations where there is an urgent need for temporary or short term care, we are able to organise accommodation and support. The placement may occur through breakdowns in care home placements, foster care or when a young person needs to be bailed from custody at the local police station. These types of placements may be required out of office hours which makes it difficult to house them immediately with most providers. the service also comes with 'responsible adult' service where a member of staff will receive the child from custody or detention over an offence that has resulted in an arrest.

Avance Care uses a robust care management system to efficiently log reports such as weeklies, daily logs, incident reports, MISPER, Risk assessments, LAC reports and much more. We are able to use the cloud-based system to set self -development targets for our young people and track progress each week using the scoring mechanism.
